Labour does not always start dramatically. For many women, especially in the first pregnancy, the early phase can be slow and confusing. The practical question is simple: when is it still safe to stay at home for a while, and when is it time to leave for the hospital?

Common Signs That Labour May Have Started
- Contractions that keep returning and gradually become stronger
- Back pain or lower abdominal tightening that comes and goes
- A show or mucus discharge with blood staining
- Waters breaking
- A feeling of pelvic pressure
Early labour can last many hours, especially in the first pregnancy. During this stage, contractions may still be irregular.
When You Should Usually Come to the Hospital
For many women, it is time to come in when contractions are:
- about 5 minutes apart
- lasting around 45 to 60 seconds
- continuing for at least 1 hour
- strong enough that you cannot comfortably walk or talk through them
If this is not your first baby, labour may move faster. In that case, come earlier rather than later.
Come Earlier Than This If Any of These Happen
- Your waters break, especially if the fluid is green, brown, foul-smelling, or heavily blood-stained
- You have vaginal bleeding more than light spotting
- Baby movements become less than usual
- You have severe abdominal pain that does not come and go like contractions
- You have high blood pressure symptoms such as severe headache, blurred vision, swelling, or chest discomfort
- You have fever, breathlessness, or feel generally unwell
- You are carrying twins or have been told your pregnancy is high risk
When It May Still Be Early Labour
You may still be in early labour if:
- contractions are irregular
- you are still comfortable between them
- they slow down with rest, hydration, or a warm bath
During this phase, many women do better resting at home, eating something light, staying hydrated, and keeping the phone nearby.
