What Could Be Causing Sudden Severe Stomach Pain?

Sudden severe abdominal pain (known as "acute abdomen" in medical terms) is one of the most common surgical emergencies. The cause needs to be identified quickly because several conditions require emergency surgery within hours. Here are the most common causes:

1. Appendicitis

The appendix becomes inflamed and infected. If untreated, it can burst within 24-72 hours, spreading infection throughout the abdomen (peritonitis) which is life-threatening.

Signs: Pain starting around the navel, shifting to the lower right abdomen. Low-grade fever, nausea, loss of appetite.

Treatment: Emergency laparoscopic appendectomy at R.K. Hospital by Dr. Rajesh Kanungo.

2. Perforated Ulcer

A peptic ulcer (in the stomach or duodenum) erodes through the full thickness of the wall, creating a hole. Acidic stomach contents leak into the abdominal cavity causing intense chemical peritonitis.

Signs: Sudden, severe "knife-like" pain in the upper abdomen. Board-like rigidity of the abdomen. Patient lies very still because any movement worsens the pain.

Treatment: Emergency surgery to close the perforation. Available 24/7 at R.K. Hospital with ICU backup.

3. Intestinal Obstruction (Bowel Blockage)

The passage of food and waste through the intestines is blocked. This can be caused by adhesions (scar tissue from previous surgery), hernias, tumours, or twisted bowel (volvulus).

Signs: Severe crampy abdominal pain that comes and goes. Complete inability to pass gas or stool. Progressive abdominal distension (swelling). Persistent vomiting.

Treatment: Initial treatment includes IV fluids, nasogastric decompression. Surgery if the obstruction does not resolve or if there are signs of strangulation. R.K. Hospital has X-ray and surgical facilities 24/7.

4. Ectopic Pregnancy (Women)

A pregnancy growing outside the uterus (usually in the fallopian tube). As it grows, the tube can rupture causing massive internal bleeding.

Signs: Severe one-sided lower abdominal pain with missed period. Vaginal bleeding or spotting. Dizziness, fainting, shoulder pain. Rapid heartbeat, pale skin.

Treatment: Emergency surgery to stop the bleeding and remove the ectopic pregnancy. Our gynaecologists Dr. Veena Pandey and Dr. Pooja Bansal, along with the surgical team, handle these emergencies at R.K. Hospital.

5. Kidney Stones (Renal Colic)

A stone moving from the kidney into the ureter causes one of the most intense pains known in medicine. While not always a surgical emergency, the pain is severe enough to bring patients to the ER.

Signs: Severe, colicky pain in the flank (side) radiating to the lower abdomen and groin. Comes in waves. Blood in urine. Nausea and vomiting.

Treatment: Pain relief, IV fluids, and ultrasonography for diagnosis at R.K. Hospital. Most stones pass with medical management; larger stones may need intervention.
