Piles vs Fissure vs Fistula: What Is the Difference?

Many patients use the word “piles” for every anal problem. But piles, fissure, and fistula are different conditions, and the treatment can be very different too. Getting the name right matters.
Piles
Piles are swollen blood vessels in or around the anus.
Common features:
- bleeding during or after passing stool
- swelling or a mass near the anus
- itching or discomfort
- sometimes pain, especially if a pile becomes thrombosed
Piles are often linked to constipation, straining, and long sitting.
Fissure
A fissure is a small tear in the anal skin.
Common features:
- severe sharp pain while passing stool
- burning pain that may continue afterwards
- small amount of bright red bleeding on tissue
- fear of passing stool because of pain
Fissure pain is often more intense than piles pain.
Fistula
A fistula is an abnormal tunnel that usually develops after an infection or abscess near the anus.
Common features:
- repeated pus discharge
- swelling or boil-like episodes near the anus
- pain and tenderness
- staining of underwear
- sometimes fever
Fistula usually does not behave like simple constipation-related pain. It often keeps coming back.
Easy Practical Difference
- Bleeding without much pain often suggests piles
- Sharp stool-related pain often suggests fissure
- Pus, swelling, or repeated abscess raises concern for fistula
But this is not perfect. One patient can also have more than one issue.
When You Should See a Surgeon
Please do not self-diagnose for months if you have:
- repeated bleeding
- severe pain while passing stool
- a lump near the anus
- pus discharge
- fever with anal pain
Rectal bleeding should never be ignored, especially if it keeps happening.
Treatment Is Not the Same for All Three
- piles may improve with stool-softening, medicines, or procedures
- fissure may improve with constipation control and medical treatment
- fistula often needs proper surgical evaluation and usually does not settle with ointment alone
