Common Causes of Lower Abdomen Pain in Women
1. PCOD / PCOS (Polycystic Ovarian Disease)
PCOD is one of the most common hormonal disorders in women of reproductive age in India, affecting approximately 1 in 5 women. The ovaries develop multiple small cysts and produce excess male hormones (androgens).
Symptoms:
- Irregular, infrequent, or prolonged menstrual periods
- Lower abdominal pain or pelvic discomfort
- Weight gain, especially around the waist
- Acne, excess facial or body hair (hirsutism)
- Difficulty getting pregnant
- Thinning hair on the scalp

2. Ovarian Cysts
Ovarian cysts are fluid-filled sacs that develop on or inside the ovary. Most are functional cysts that form during the menstrual cycle and resolve on their own. However, some cysts can grow large or cause complications.
Symptoms:
- Dull or sharp pain on one side of the lower abdomen
- Feeling of fullness, bloating, or pressure in the pelvis
- Pain during menstruation that is worse than usual
- If ruptured: sudden severe pain, nausea, dizziness (emergency)
Diagnosis: Ultrasonography at R.K. Hospital clearly shows the cyst size, type, and location. Most small cysts are monitored. Large or suspicious cysts may need laparoscopic removal.
3. Urinary Tract Infection (UTI)
UTIs are extremely common in women due to the shorter urethra. The infection can affect the bladder (cystitis) or, if untreated, spread to the kidneys (pyelonephritis).
Symptoms:
- Burning or stinging sensation during urination
- Frequent urge to urinate with small amounts passed
- Lower abdominal pain or pressure above the pubic bone
- Cloudy, foul-smelling, or bloody urine
- Fever and back pain if infection reaches the kidneys
Treatment: Urine analysis and culture at our pathology lab (Dr. Apoorva Tripathi, MBBS, MD) identifies the bacteria. A course of antibiotics prescribed by our doctors provides relief within 1-3 days.
4. Ectopic Pregnancy (Emergency)
An ectopic pregnancy occurs when a fertilized egg implants outside the uterus, most commonly in the fallopian tube. This is a medical emergency as the growing pregnancy can rupture the tube, causing life-threatening internal bleeding.
Warning signs:
- Sharp, severe pain on one side of the lower abdomen
- Missed period with abnormal vaginal bleeding or spotting
- Shoulder tip pain (a referred pain sign)
- Dizziness, fainting, or feeling light-headed (sign of internal bleeding)

5. Menstrual Disorders (Dysmenorrhoea)
Many women experience painful periods (dysmenorrhoea). While mild cramps are normal, severe pain that interferes with daily life may indicate an underlying condition like endometriosis, fibroids, or adenomyosis.
When to seek help:
- Menstrual pain that does not respond to painkillers
- Progressively worsening period pain over months/years
- Very heavy periods (changing pads every 1-2 hours)
- Bleeding lasting more than 7 days
- Bleeding between periods
