Piles, fissure and fistula: which problem is it?
Piles, anal fissure and anal fistula are different conditions. Bleeding, pain or discharge from the anus needs a diagnosis before treatment is chosen. Some piles and fissures improve with conservative care; a persistent fistula usually needs a surgeon’s assessment.

Plan stool care and follow-up
The sequence explains planned care. Urgent symptoms require prompt assessment and may change this pathway.
What symptoms and history should I describe?
Piles are enlarged haemorrhoidal tissue, a fissure is a tear, and a fistula is an abnormal channel that often follows an abscess. Bleeding with bowel movements, sharp pain during stool, and recurring pus or swelling suggest different possibilities, but none is diagnostic on its own.
Tell the doctor about constipation, diarrhoea, how much blood you have seen, whether pain lasts after passing stool, and any discharge or fever. Mention weight loss, changed bowel habits, family history of bowel disease and previous treatment. Do not label all rectal bleeding as piles.

How is the problem assessed?
The assessment may include an examination of the anal area and further testing if the source of bleeding or pain is uncertain. Discuss discomfort, privacy and a chaperone before examination. Tests are selected according to the findings; not everyone needs the same scan or endoscopy.
For a suspected fistula, the position of the tract and its relationship to the sphincter muscles matters. Ask whether an abscess needs drainage and whether imaging is necessary. A recurrent problem may require a different plan from a first episode.

What treatment and follow-up might be discussed?
Treatment for uncomplicated piles may begin with stool-softening measures and changes that reduce straining. Persistent symptoms can require an office procedure or operation. The right option depends on the diagnosis and severity, rather than whether an advertisement calls it laser or painless.
Fissures may be treated with measures to keep stool soft and prescribed local treatment. A non-healing or recurrent fissure may need a procedure. A fistula often needs surgical treatment; the clinician should explain how the plan aims to heal the tract while protecting continence.
Ask what level of pain, bleeding or discharge is expected after treatment, how dressings will be managed, and when to seek review. Do not combine multiple over-the-counter creams indefinitely. Record what you have used and whether it helped, so the next consultation builds on a clear history.

Warning signs → urgent assessment
Heavy bleeding with dizziness or fainting, black stools, fever with a painful anal swelling, or rapidly worsening pain needs urgent assessment.
Stable concern → planned consultation
Bring a symptom timeline and relevant reports. The clinician reviews the concern, chooses indicated tests and explains treatment or referral. Arrange follow-up for a changing or persistent problem.
Questions to ask at the appointment
- Is the diagnosis piles, fissure, fistula or another cause of bleeding?
- What conservative treatment is reasonable before a procedure?
- How does the proposed procedure affect continence and recurrence risk?
- What aftercare and follow-up does this procedure require?
Take a short written list and begin with your most important concern. If a test or treatment is recommended, ask what decision it will help make and how the result will be communicated. Bring the plan back at follow-up so you can discuss what improved and what remains unresolved.
