Creatinine Normal Range — Serum Creatinine, BUN and eGFR

Understand your Serum Creatinine Test report — normal values, what abnormal results mean, and when to get tested.

What is Serum Creatinine Test?

Creatinine is a waste product made by the muscles at a steady rate every day and removed from the blood by the kidneys. Because production is constant, the blood creatinine level is a reliable mirror of how well the kidneys are filtering — when kidney function drops, creatinine rises. It is the single most important kidney test and is used to calculate eGFR, which stages chronic kidney disease. Creatinine is checked routinely in diabetes, high blood pressure, before CT scans with contrast, and before starting many medicines. Because it depends on muscle mass, normal values differ for men, women, children, and the elderly.

What Do Abnormal Results Mean?

Serum Creatinine — Men

HIGH

Reduced kidney function, dehydration, high protein diet, muscle injury, medicines (NSAIDs, some antibiotics)

LOW

Low muscle mass, liver disease, malnutrition, pregnancy

Serum Creatinine — Women

HIGH

Reduced kidney function; women have lower muscle mass so even 1.2 may be abnormal

LOW

Low muscle mass, pregnancy (normal), liver disease

Serum Creatinine — Children (1–12 years)

HIGH

Kidney disease, dehydration, urinary obstruction

LOW

Normal in small children

Serum Creatinine — Elderly (above 65)

HIGH

Note: elderly have less muscle, so a "normal" creatinine may hide reduced kidney function — eGFR is more accurate

LOW

Low muscle mass

Blood Urea Nitrogen (BUN)

HIGH

Kidney disease, dehydration, high protein intake, GI bleeding, heart failure

LOW

Liver disease, malnutrition, overhydration

BUN : Creatinine Ratio

HIGH

Above 20:1 = dehydration, GI bleeding, heart failure (pre-renal cause)

LOW

Below 10:1 = liver disease, malnutrition, muscle injury

eGFR (calculated from creatinine)

HIGH

Not usually a concern

LOW

Stage 2: 60–89 (mild). Stage 3a: 45–59, Stage 3b: 30–44 (moderate). Stage 4: 15–29 (severe). Stage 5: below 15 (kidney failure, dialysis)

Creatinine Clearance (24-hr urine)

HIGH

Pregnancy, high protein diet

LOW

Reduced kidney function

How to Prepare for Serum Creatinine Test

  • Fasting for 8-10 hours is preferred (not strictly required)
  • Avoid heavy exercise and large meat meals for 24 hours before the test
  • Drink normal amounts of water — dehydration raises creatinine
  • Inform your doctor about painkillers (NSAIDs), BP medicines, or supplements like creatine

When Should You Get Tested?

  • Diabetes — at least once a year
  • High blood pressure
  • Swelling of feet, ankles, or around the eyes
  • Decreased urine output or foamy urine
  • Before CT scan with contrast dye
  • Before starting or adjusting medicines cleared by the kidney
  • Long-term painkiller use
  • Family history of kidney disease, or age above 60

Frequently Asked Questions

What is the normal creatinine level?

Normal serum creatinine is 0.7–1.3 mg/dL for men and 0.6–1.1 mg/dL for women. For children it is 0.3–0.7 mg/dL. Because creatinine depends on muscle mass, doctors also calculate eGFR (normal above 90) for an accurate picture of kidney function.

Creatinine 1.5, 2, or 3 — how serious is it?

Creatinine 1.3–1.5 mg/dL is mildly raised — often from dehydration, painkillers, or early kidney disease; it needs a repeat test and eGFR. Creatinine 2–3 mg/dL usually means moderate kidney disease (eGFR roughly 25–40) and needs a nephrologist. Creatinine above 4–5 mg/dL suggests severe kidney failure, and dialysis may be discussed depending on symptoms and eGFR.

Can creatinine be reduced?

If the cause is dehydration, painkillers, or infection, creatinine can return to normal once treated. In chronic kidney disease, creatinine cannot be "reversed" but can be kept stable by controlling blood sugar and blood pressure, avoiding NSAIDs, limiting salt and excess protein, and regular follow-up. No home remedy lowers creatinine reliably.

What is the difference between creatinine and eGFR?

Creatinine is the raw blood level. eGFR (estimated Glomerular Filtration Rate) is calculated from creatinine plus age and sex, and tells the percentage of kidney function remaining. eGFR is more accurate — for example, creatinine 1.2 can mean normal kidneys in a muscular young man but Stage 3 kidney disease in an elderly woman.
